Contractor Monthly HSE and IVMS Report

The monthly report an oil and gas contractor presents to the client: work hours, recordable and lost time injuries, vehicle crashes, near misses, IVMS driver colors and events, inspections and open actions. The Excel workbook calculates rolling 12-month TRIR, LTIF and crash rate against the client's targets; the 11-slide PowerPoint deck presents them with native charts you can edit.

Free: enter your name, email and job title and we email you the file. The deck opens in PowerPoint, Google Slides and LibreOffice Impress; the workbook works in Excel, Google Sheets and LibreOffice Calc, with no macros.

What's in the files

One workbook for the numbers, one deck for the meeting

Add the month to the workbook, read the KPIs against the client's targets, then copy the figures into the deck and replace the text in square brackets.

  1. 01

    Excel: monthly data

One row per month: work hours, cases by type, first aid, near misses, days lost, distance, crashes, five IVMS event types, drivers in green, amber and red, inspections and toolbox talks. Rolling 12-month TRIR, LTIF and crash rate calculate.

  2. 02

    Excel: KPIs

The report month, rolling 12 months and year to date for 12 indicators, each against the client's target with a status, plus two charts.

  3. 03

    Excel: actions and settings

An action log with source, person responsible, due date, days open and an overdue flag, and the settings for company, client, contract, report month and targets.

  4. 04

Deck: the month and the rates

Title, the month at a glance in six tiles, rolling injury rates against target, and vehicle crashes with the rolling crash rate.

  5. 05

    Deck: IVMS

Drivers in green, amber and red by month, the drivers in red and the coaching booked, and IVMS events by type this month against last month.

  6. 06

    Deck: incidents, actions, next month

Incidents and near misses, inspections and toolbox talks, open actions, three priorities for next month with what you need from the client, and the definitions slide.

What is a contractor monthly HSE report?

Oil and gas clients ask each contractor for a monthly HSE report and review it in a monthly meeting. The report shows the hours worked, the injuries and their rates, vehicle crashes and the distance driven, near misses, the IVMS results of the drivers, inspections done and the actions still open. Most clients compare contractors on the same few rates, so a contractor that brings them clearly and on the same basis every month has an easier meeting.

This template splits the work in two: an Excel workbook that holds the monthly data and does the arithmetic, and a PowerPoint deck that tells the story of the month with editable charts.

How to use the report

  1. Set it up once: on Settings, enter the company, client, contract and the client's targets.
  2. Add the month: hours and cases from the incident register; distance, crashes, IVMS events and driver colors from your tracking reports; inspections and toolbox talks.
  3. Read the KPIs: month, rolling 12 months and year to date, each against the target.
  4. Update the actions: what was closed, what is open and what is overdue.
  5. Fill the deck: paste the figures into the charts' data (right click, Edit data), replace every text in square brackets and delete the "Example data" tags.

How are the rates calculated?

LTI = fatalities + lost work day cases TRI = LTI + restricted work cases + medical treatment cases Rolling TRIR = TRI in the last 12 months ÷ their work hours × 1,000,000 Rolling LTIF = LTI in the last 12 months ÷ their work hours × 1,000,000 Rolling crash rate = crashes in the last 12 months ÷ their km × 1,000,000 IVMS events per 1,000 km = events of the month ÷ km of the month × 1,000

To try the same rates on your own figures, or to see how many hours without a case bring a rate back under the target, use the LTIF, TRIR & Vehicle Crash Rate Calculator.

Worked example: October 2026

  • A contractor of about 850 people and 160 vehicles worked 210,000 hours and drove 420,000 km in October, with no recordable case and no crash.
  • Over the last 12 months it had 4 recordable cases and 2 lost time injuries in 2,365,000 hours: rolling TRIR 1.69 and LTIF 0.85, both above the example targets of 1.50 and 0.80.
  • 4 crashes in 4,730,000 km give a rolling crash rate of 0.85 per million km against an example target of 0.60.
  • IVMS: 3 of 150 drivers in red (2.0%), inside the example limit of 3%, and 1.07 IVMS events per 1,000 km.
  • One of four actions is overdue: rollover protection on LV-102, due 25 October.

Common mistakes in monthly HSE reports

  • One month's rate as the headline. A month with no case shows 0.00; the client looks at the rolling rate.
  • Changing the basis. Per 200,000 hours one month and per million the next makes the trend meaningless.
  • Hours and cases for different people. Count the same staff and subcontractors in both.
  • IVMS figures with no action. Name the drivers in red and the coaching booked, not only the count.
  • Actions without dates. An action with no due date is never overdue and never closed.

What the template does not do

The template follows no single client's report format; add the slides and indicators your client asks for. Case definitions and targets are your client's. The workbook does not read your incident register or tracking system: you copy the figures in each month. For the driver ratings themselves, see the IVMS Contractor Compliance Kit; for a general fleet operations review, the Monthly Fleet Review Deck.

FAQ

Questions about monthly HSE and IVMS reports

Want the IVMS figures ready every month? Talk to Safee.

What should a monthly HSE report for an oil and gas client include?

Work hours, injuries by type and their rates, vehicle crashes and distance, near misses, IVMS driver results, inspections and training, open actions and next month's priorities. The client's own format decides the rest.

Why report the rolling 12-month rate?

Because one month has too few hours: a single case moves the month's rate a lot, and a month with no case shows zero. The rolling rate shows the trend the client cares about.

Where do the IVMS figures come from?

From your tracking system's IVMS reports: event counts by type and the number of drivers in each color for the month.

Can I change the targets?

Yes. The targets on Settings are examples; enter those in your contract or HSE plan.

Are the charts in the deck editable?

Yes. They are native PowerPoint charts: right click a chart and choose Edit data to paste your figures.

Do the files work in Google Slides and Google Sheets?

Yes. Upload them to Google Drive and open them there. They also work in PowerPoint, Excel and LibreOffice, with no macros.
