What is a vehicle accident report form?
A vehicle accident report form is the company's own record of an accident, filled in by the driver at the scene or as soon as possible after. It does not replace the official report from Najm or the police. It collects what the company needs from the same moment: who was involved, what happened, the official case number, photos and witnesses, and later the supervisor's review and the insurance claim.
Details fade within hours. A driver who has a printed form in the vehicle writes down plate numbers, insurers and witness phones before people leave, and that is usually what a claim depends on.
Who handles the accident report in Saudi Arabia and the UAE?
Section 5 of the form records which authority handled the accident and its reference number. As of October 2026, published guidance describes the process as follows. Check the current procedure with the authority and your insurer.
| Country | Who handles it | Source |
|---|---|---|
| Saudi Arabia | Najm Insurance Services handles accidents where at least one vehicle is insured, by phone or through the Najm app, and sends the report used for the insurance claim. The traffic department (Muroor, 993) handles serious accidents and cases where no vehicle is insured. Medical help: 997. | Gulf News, September 2024 |
| UAE | Report to the police (999). In Dubai, minor accidents with no injuries can be reported through the Dubai Police app. Insurers ask for the police report. | Shory, November 2025 |
The form is not an official Najm, Muroor or police form, and it is not legal advice. Its job is to make sure the company has the case number and the facts behind it.
How to fill in an accident report, step by step
- Keep a blank form in every vehicle, in the glove box with the registration and insurance. The vehicle handover form includes it in the documents check.
- Follow the scene checklist: make the scene safe, check for injuries, report the accident and call the supervisor.
- Fill in sections 1 to 10 while details are fresh: accident, parties, injuries, official report, damage, photos, sketch, witnesses and statement.
- Review within a day. The supervisor interviews the driver, checks tracking data and dashcam footage, and records preventability and a corrective action.
- Track the claim and log it. Record each claim step, then enter the accident in the fleet incident and claims log.
What photos should a driver take after an accident?
The photos checklist has ten items, from all vehicles at four corners to the tow truck. The ones most often missed are the final positions of the vehicles before they were moved, a wide view of the road with its signs and lane markings, and the plates of every vehicle. Take them only when it is safe and allowed, and photograph people or their documents only with their consent.
The sketch box asks for the roads and lanes, direction of travel, the vehicles marked A (company vehicle), B and C, the point of impact, signs or signals, and north. A rough sketch drawn at the scene is worth more than a neat one drawn a week later.
Worked example: a rear-end accident at a signal
At 07:40 a company pickup is hit from behind while stopped at a red light. Nobody is hurt. The driver switches on the hazard lights, places the triangle, reports the accident to Najm through the app and calls his supervisor.
- Section 3: Vehicle B's driver, plate, insurer and policy number.
- Section 5: reported to Najm, case number recorded, liability in the report: our driver 0%, other party 100%.
- Sections 6 and 7: rear area "D – rear bumper and tailgate", 14 photos saved under the report number.
- Section 11: tracking data shows the vehicle stationary at the time of impact; the supervisor marks the accident not preventable.
- Section 12: insurer notified the same day; the vehicle is off road for 6 days; the claim is closed when the settlement arrives.
Common accident reporting mistakes
- Agreeing on blame or payment at the scene. Leave liability to the official report and the insurer.
- Driving away without the case number. Without it the claim cannot move.
- Missing the other party's insurer and policy number. They are hard to get once everyone has left.
- Photos only of your own damage. The scene, the other vehicle and its plate matter as much.
- A statement written days later. Ask for it the same day, in the driver's own words.
- Closing the form when the vehicle is repaired. Close it when the claim is settled and the corrective action is done.
What the accident form cannot do
The form records what the driver and the supervisor report. It cannot confirm speed, location or harsh braking before the impact; tracking data and dashcam footage can. It is not an official Najm, Muroor or police form and does not replace their report. Procedures, emergency numbers and apps change, so check the current rules with the authority and your insurer. It is a working template, not legal advice.
