Five sheets, from the roadside to the review

The form is filled in at the scene, the log follows the case until it closes, and the summary shows the patterns across the year.

  1. 01

    Incident log

Date, vehicle, driver, location, type, injuries, third party, police and claim numbers, costs, recovery, back-in-service date, root cause and action. Net cost, days off road and status are calculated.

  2. 02

    Summary

Incidents, net cost, average days off road, open incidents and open claims for the year, by type and by month.

  3. 03

    Accident report form

Six steps at the scene and fourteen details to record. Print it and keep a copy in every vehicle.

  4. 04

    Breakdown procedure

Six steps for the driver and the control room when a vehicle breaks down on the road.

  5. 05

    Settings

Vehicles, drivers, currency and the year shown on the summary.

What is a fleet incident log?

A fleet incident log records every accident, damage, theft, breakdown and injury involving your vehicles, and follows each one until it is closed. It answers the questions that come up later: what happened, what it cost after the insurer paid, how long the vehicle was off the road, and what changed so it does not happen again.

This fleet incident log template in Excel keeps those answers in one row per incident, adds a yearly summary by type and month, and includes an accident report form for every vehicle and a roadside breakdown procedure.

How to use the fleet incident log

  1. Add vehicles and drivers on the Settings sheet.
  2. Open a row as soon as an incident is reported, before costs are known. Record date, vehicle, driver, location and type.
  3. Add the police report and claim numbers and the claim status as they arrive.
  4. Record repair and other costs, such as towing or a rental vehicle, and any amount recovered from the insurer or a third party.
  5. Enter the back-in-service date, the root cause and one action to prevent a repeat.
  6. Review the Summary monthly and keep the Accident report form printed in each vehicle.

How are net cost and days off road calculated?

Net cost = repair cost + other costs − recovered Days off road = back in service − incident date (or today − incident date while the vehicle is still off the road) Status = Closed when the vehicle is back in service and the claim is not open; otherwise Open

In the example, a collision cost 3,200 in repairs and 450 in other costs, and 2,700 was recovered from the insurer, for a net cost of 950. The vehicle was off the road for 7 days.

How to find the root cause and prevent a repeat

The last two columns matter most. A root cause is the reason behind the event, not just a description of it: "following distance too short" rather than "rear-end collision". The action should be one specific step, such as coaching on following distance, a guide when reversing in yards, or a new check in the service schedule. Reviewing these columns each month shows which causes repeat.

What goes in the in-vehicle accident report form?

The form lists six steps at the scene, from making the area safe to calling the police where required, and fourteen details to record: time, place, vehicles, other driver and insurer, witnesses, injuries, photos and signatures. Drivers should never admit fault at the scene; the form says so.

What to do when a vehicle breaks down on the road

A breakdown is an incident too. The procedure sets out what the driver does (get safe, report, protect the load, hand over to recovery) and what the control room does (decide on repair, tow or replacement, tell the customer, book the workshop and record the cost).

Limits of a spreadsheet incident log

The log depends on prompt and complete reporting. It does not replace police, insurer or legal procedures, and it does not show what happened before the event, such as speed or harsh braking; that needs vehicle data.

What should a fleet incident report include?

Date, time and place; vehicle and driver; what happened; damage and injuries; other parties, their insurer and witnesses; police report number; photos; and signatures. The template's accident report form lists all of these.

What is the difference between an incident log and a claims log?

An incident log records every event, including those never claimed. A claims log follows insurance claims. This template does both: each incident has a claim number and status, so you can see claimed and unclaimed costs together.

How do I calculate the true cost of a vehicle accident?

Add repair costs and other costs such as towing, a rental vehicle and admin time, then subtract what was recovered. The ROI calculator on Safee's tools page has an incident cost tab for a fuller estimate, including lost work.

When is an incident closed?

In the template, when the vehicle is back in service and the claim is settled, rejected or not made. Until then the row stays open and counts in the summary.

Should breakdowns go in the incident log?

Yes. They cost money and time off the road, and their root causes often point to gaps in maintenance. The template has a "Breakdown on the road" type and a procedure for it.

How often should incidents be reviewed?

Look at open incidents weekly and the Summary monthly, focusing on the root cause and action columns to find causes that repeat.

Does the template work in Google Sheets?

Yes. Upload the .xlsx file to Google Drive and open it with Google Sheets. It also works in Excel and LibreOffice Calc, with no macros.

Does the log replace the insurer's or police process?

No. It is your internal record. Follow your insurer's claims process and the reporting rules in your country.
