A fleet risk register and a written road safety program

The register shows which road risks matter most and what controls them. The program writes down the rules and who is responsible, and refers to the register.

  1. 01

    Risk register

Hazard, factor, who is exposed, existing controls, likelihood and severity, extra controls by type, responsible, due date, residual score, monitoring KPI, data source and review date.

  2. 02

    Risk matrix

Editable bands and what each requires, rating scales, and two 5×5 grids that count your risks in each cell before and after controls.

  3. 03

    Driver risk profile

Km, night km share, events per 100 km, incidents, fines and training for each driver, turned into risk points and a band with weights you set.

  4. 04

    Route hazards

Regular routes with distance, road type, night travel, known hazards, rest points and coverage gaps, with a note where tracking should not rely on mobile coverage.

  5. 05

    Safety KPIs and action plan

Twelve months of exposure, outcomes and compliance, with crashes, lost-time injuries and fatigue violations per 1,000,000 km and two charts; actions tracked to done and effective.

  6. 06

    Word: road safety program

Policy statement, scope including grey fleet, objectives, roles, risk method, core rules, drivers, vehicles, contractors, incidents, KPIs, management review, approval and an ISO 39001 cross-reference.

What is a fleet risk assessment?

A fleet risk assessment lists the ways your vehicles and drivers could cause harm on the road, rates each one, and records the controls that bring it down to an acceptable level. It covers the whole operation: speed, fatigue, night and desert journeys, loads, yards and site gates, weather, and the people around your vehicles.

It is different from a journey management plan, which assesses one journey before it starts. This register is company-wide and reviewed on a set date; the journey plan applies its rules to each trip.

Who needs a fleet risk assessment and road safety program?

  • HSE and fleet managers who need a written road safety program for management or a client.
  • Contractors asked for a road risk assessment in a prequalification or tender.
  • Companies whose drivers use private cars for work, the "grey fleet", which a program should cover too.

IOGP notes in Report 365 that driving-related incidents have historically been the single largest cause of fatalities in its member companies' operations, which is why energy clients ask for a structured approach to road risk.

How to use the fleet risk assessment template

  1. Check the risk matrix and match the bands to your company matrix.
  2. List the risks, one per row, choosing the factor from the ISO 39001 list so no area is missed.
  3. Add controls for High and Extreme risks in order of preference: eliminate, substitute, engineering, administrative, protective equipment. Rate the residual risk.
  4. Link each risk to a KPI and its data source, such as night km share from the tracking report.
  5. Profile drivers and routes to decide who to coach first and which routes need a journey plan.
  6. Fill in the Word program, have it signed, and review both files yearly and after any serious incident.

How is fleet risk scored?

Risk score = likelihood (1–5) × severity (1–5) Level (example) = Low 1–4 · Medium 5–9 · High 10–19 · Extreme 20–25 Residual score = likelihood after controls × severity after controls Rate per 1 M km = count in the month ÷ km in the month × 1,000,000

The bands are the same as in Safee's journey management plan template, so a hazard rated in both gets the same level. The driver risk profile adds points for night driving, events per 100 km, incidents, fines and training that is not current; its weights and limits are your choices on the Settings sheet.

Worked example: sandstorms on desert roads

  • Before extra controls: likelihood 4, severity 5, score 20, Extreme: the activity should not continue as it is.
  • Extra controls: stop trips on a visibility warning and a written safe-stop procedure. After controls: likelihood 2, severity 5, residual 10, High, so the operations manager approves and the risk stays on the review list.
  • KPI: trips stopped for weather, from the incident log. In the example KPIs, September had 1 crash over 415,000 km = 2.41 crashes per 1,000,000 km; January to September: 12 crashes over 3,585,000 km = 3.35.

Common fleet risk assessment mistakes

  • Training as the only control. Look first at removing or changing the journey, then at engineering.
  • No residual rating. Without it you cannot tell whether the controls are enough.
  • Risks with no KPI. A control you do not measure tends to fade.
  • Forgetting the grey fleet and subcontractors. Their crashes are still your road risk.
  • A register written once. Review it on its date, after serious incidents and when work changes.

Limits and sources

The template is a generic working tool. The factor list and KPI groups follow the road traffic safety performance factors of ISO 39001:2012, and the fatality statement comes from IOGP Report 365. It does not claim to meet ISO 39001 or any client's standard, and it sets no speed or driving-hour limits: those come from your authority or company policy, so check your authority's current requirement. Driver profile weights and color limits are company choices, not a standard.

Measure the controls with tracking data

Most of the intermediate KPIs in this register come from driving data. Safee Alarms and Alerts covers more than 50 alarm types, and Fleet Reporting turns them into monthly reports.

  • Speed, seatbelt and out-of-working-hours alarms for speed, seatbelt and night km KPIs.
  • Continuous and accumulative driving alarms for fatigue violations.
  • Journey Management (JMS) for journey plan compliance.
  • Satcomm for routes without mobile coverage.

Questions about fleet risk assessment

What should a fleet risk assessment include?

Each hazard with who is exposed, where and when, existing controls, likelihood and severity, extra controls, a responsible person and date, the residual risk, a KPI to monitor and a review date. The template adds a matrix, a driver risk profile, route hazards and monthly KPIs.

How do you calculate the risk score?

Likelihood from 1 to 5 multiplied by severity from 1 to 5, before and after controls. The example bands are Low 1–4, Medium 5–9, High 10–19 and Extreme 20–25, editable on the Risk matrix sheet.

What is a road safety program?

A written document that sets the company's road safety policy, scope, objectives, roles, rules, driver and vehicle standards, incident handling, KPIs and management review. The Word file in this template has 13 sections and an ISO 39001 cross-reference.

Does the template meet ISO 39001?

No. It uses the ISO 39001 performance factors to organize risks and KPIs, but using it does not make a company compliant or certified.

How is it different from the journey management plan?

The journey management plan assesses one journey before it starts. This register covers the company's road risks as a whole and is reviewed on set dates.

Does it cover private cars used for work?

Yes. The program's scope includes the grey fleet, and you can list grey-fleet risks in the register like any other.

Does it work in Google Sheets and Google Docs?

Yes. Upload the .xlsx and .docx files to Google Drive and open them there. They also work in Excel, Word and LibreOffice, with no macros.
