Field Service Job Sheet and Visit Log

A job sheet the technician takes to every visit: job details, assignment, visit times, a safety check, work done, parts, result and the customer's signature. The Excel visit log puts the times the technician wrote next to the arrival and departure times from tracking, and calculates response time, time on site and first-time fix.

Free: enter your name, email and job title and we email you the file. The job sheet opens in Word, Google Docs and LibreOffice; the log works in Excel, Google Sheets and LibreOffice Calc, with no macros.

What's in the file

A job sheet for the technician and a log for the office

The office opens the job and assigns it, the technician completes the sheet on site and the customer signs it. The office then checks the times against tracking and adds the visit to the log.

  1. 01

Job sheet: job, equipment and assignment

Job and visit number, request time, customer, site, contact, job type, priority and the response target in the contract. Then the equipment, serial number, cover, the reported problem, technician, vehicle and arrival window.

  2. 02

    Job sheet: visit times

Left base, arrived, work started, work finished and left site as the technician recorded them, with odometer readings, plus two columns the office fills from the tracking report.

  3. 03

    Job sheet: safety before work

Eight checks before the technician starts: site sign-in, permit to work, PPE, hazards, isolation and lock-out, safe parking, telling the customer, and stopping when the work is not safe.

  4. 04

Job sheet: work, result and sign-off

Fault and cause, work carried out, parts used with their source, labor and travel time, the result, the reason if not completed, a follow-up visit, the customer's signature and rating, and an office section.

  5. 05

    Excel: visit log

One row per visit. It calculates the response target and time, whether the target was met, the arrival and departure difference from tracking, time on site and first-time fix, with seven flags.

  6. 06

    Excel: summary and settings

Visits, completed jobs, target met %, first-time fix %, response time, time on site and flags by month, technician and priority. Settings for response targets, the time allowance and lists.

What is a field service job sheet?

A field service job sheet, also called a service report or job card, is the record of one visit by a technician to a customer site. It says what was asked, who went and in which vehicle, when they arrived and left, what they found and did, which parts they used, whether the job is finished, and it carries the customer's signature.

The sheet is the basis for the invoice and for any dispute about the visit. For that reason the times on it matter: a customer who questions an arrival time or the hours charged will compare them with their own records. The visit log in this template compares them with tracking first.

How to use the job sheet and visit log

  1. The office opens the job: request time, customer, site, job type, priority and the response target from the contract.
  2. The office assigns it to a technician and vehicle with an arrival window agreed with the customer.
  3. The technician records each time as it happens, completes the safety check before starting and the work, parts and result at the end.
  4. The customer signs before the technician leaves. If they refuse or are not there, the technician writes why.
  5. The office checks the times against tracking and enters the visit in the Excel log.
  6. A second visit to the same job gets a new sheet and a new row with visit number 2, so first-time fix stays accurate.

Why compare the technician's times with tracking?

Times written at the end of the day are often rounded or estimated. A tracking system records when the vehicle entered and left the geofence drawn around the customer's site. Putting the two side by side shows three things: visits where the written time is far from the tracked time, visits that do not appear in tracking at all, and the real time spent on site, which is what you bill and plan with.

A difference is a question, not proof of a mistake. The geofence may be larger or smaller than the site, the technician may park far from the entrance, or the job may have needed a stop to collect parts. The log flags the visit and the supervisor asks.

How does the visit log calculate response time and first-time fix?

Arrival used = tracked arrival, or the technician's arrival if not tracked Response time (h) = (arrival used − request received) × 24 Target met = response time ≤ target for the priority Arrival difference = (technician arrival − tracked arrival) × 1440 minutes Time on site (min) = (departure − arrival) × 1440, for each source First-time fix = Yes when visit no. 1 has the result Completed Flag "Arrival differs" = |arrival difference| > allowed minutes

The other flags are a departure difference above the allowance, a visit with a technician arrival but no tracked arrival, a missed response target, an arrival outside the agreed window, a job not completed with no reason, and a completed job with no customer signature. The response targets of 4, 8, 24 and 72 hours by priority and the 15-minute allowance are example values on the Settings sheet; replace them with your contract terms.

Worked example: one job, two visits

Job JS-2026-303 is an emergency call-out to a mall. The request arrived at 22:40, and the response target for Emergency is 4 hours in the example.

  • The technician wrote an arrival of 01:20; tracking shows the van entering the site geofence at 01:16. Response time: 01:16 − 22:40 = 2.6 hours, within the target. The 4-minute difference is inside the 15-minute allowance.
  • The job needed a part, so the result was Not completed with the reason Parts needed and a follow-up visit. First-time fix: No.
  • Visit 2 two days later completed the job. Tracking shows 09:38 to 11:14 on site, 96 minutes; the technician wrote 09:40 to 11:10.
  • On job JS-2026-302 the technician wrote 08:30 to 11:30, but tracking shows 09:12 to 10:41. Both differences exceed 15 minutes, so the row is flagged for the supervisor.

Common mistakes with field service job sheets

  • Writing times at the end of the day. Rounded times make the response and on-site figures meaningless.
  • One sheet for several visits. A second visit hides inside the first, and first-time fix looks better than it is.
  • "Not completed" with no reason. Without the reason you cannot tell a missing part from a closed site.
  • No customer signature. An unsigned sheet is weak support for an invoice.
  • Parts written as "various". Part numbers and quantities keep van stock and invoices right.
  • Skipping the safety check on short jobs. Isolation and permits matter on a 20-minute job as much as on a full day.

What the template does not do

The log works with the times you copy into it; it does not read tracking data. Response targets, charges and what counts as a completed job come from your customer contracts, not from the template. Site safety rules, permits to work and isolation steps differ by customer and industry, so add the site's own requirements to section 5. For workshop repairs on your own vehicles, use the Maintenance Work Order Template instead. To size the team and the cost per visit, use the Field Service Visit Calculator.

Questions about field service job sheets and visit logs

Want arrival and departure times for every customer site without asking the technicians? Talk to Safee.

What should a field service job sheet include?

Job number, customer and site, job type and priority, equipment and serial number, the reported problem, technician and vehicle, arrival and departure times, a safety check, the fault and work done, parts used, the result, any follow-up visit and the customer's signature.

What is the difference between a job sheet and a work order?

Many companies use the terms for the same thing. In this template the job sheet records a visit to a customer site. For repairs to your own vehicles in the workshop, the maintenance work order is the better fit.

How is first-time fix rate calculated?

The number of jobs completed on the first visit divided by the number of first visits with a result. A job that needs a second visit counts against it, which is why each visit has its own row and visit number.

How is response time measured?

From the time the request was received to the time the technician arrived on site. The log uses the tracked arrival when there is one, because it is recorded by the system rather than written by hand.

What if the technician's time differs from tracking?

The log flags differences above the allowance on Settings. The supervisor asks why: the geofence may not match the site, the vehicle may have parked elsewhere, or the time may have been written later from memory.

Can I use the job sheet without a tracking system?

Yes. Set Vehicles are tracked to No on Settings and leave the Tracking columns empty. The log then uses the technician's times for response time and time on site.

Do the files work in Google Docs and Google Sheets?

Yes. Upload the .docx and .xlsx files to Google Drive and open them there. They also work in Word, Excel and LibreOffice, with no macros.
